Please keep in mind that RAF offers various types of memberships, including ONLINE COACHING. We’d be happy to provide you with additional information should ONLINE COACHING or another membership be of interest.
By completing this MEMBERSHIP CANCELLATION REQUEST form, you understand and agree that this will serve as your 30-day written request to cancel your membership as required by your Membership Agreement. Your membership will be cancelled thirty (30) days from the submission date of your Membership Cancellation Request form. You understand and agree that if you have a scheduled payment within this period, payment will be processed as scheduled. All payments are non refundable.
You understand and agree that termination of your membership before the end of your contract term will require a Termination Fee of the outstanding balance owed per your contract.
If you decide to reactivate your membership in the future, please keep in mind that membership rates at the time of reactivation will apply as membership rates are subject to change.
